I read with great interest the article titled "Efficacy of Auralgan for Treating Ear Pain in Children With Otitis Media" in the July 1997 issue of the ARCHIVES, by Hoberman et al¹ I have always been reluctant to use anything in an acutely painful ear. I am fearful of 2 things:

• In the unexamined ear, which may be otitis externa, either Auralgan otic solution (a combination product of antipyrine, benzocaine, and oxyquinolone sulfate dissolved in dehydrated glycerin [Wyeth-Ayerst, Philadelphia, Pa]) or olive oil may complicate the problem.
  
• In the case of acute otitis media the use of either substance, especially olive oil, which enters the middle ear cavity through a perforation that is present or pending, could conceivably create an inflammatory or granulomatous condition with chronic or permanent sequelae.
